CamSur skeds mobile passport service
PILI, Camarines Sur --- The Department of Foreign Affairs (DFA), in partnership with the provincial government of Camarines Sur under the dynamic leadership of Gov. Migz Villafuerte through Camarines Sur Employment Center (CamSur PESO) will bring back the Mobile Passport Service (MPS) to the province on June 07 & 08, 2018 at the Camarines Sur Convention Center, Capitol Complex, Cadlan, Pili in response to the growing clamor from the provincial constituents seeking access to faster passport processing without traveling great distance to the DFA regional office in Albay. Thus, the MPS is revived to accommodate the mounting requests as well as de-clog the online appointments that clutter the digital system and processing. For this purpose, the Office of Consular Affairs issued these guidelines to facilitate the process especially in the submission of application forms and basic requirements not later than May 18, 2018. Some of the said guidelines are as follows: 1. Passport applicants must submit an original duly accomplished application form and photocopies of the basic requirements to CamSur PESO. The application packets should be enclosed in a short brown envelope with the name of the applicant displayed on the upper left hand corner of the envelope in all caps and with the surname first, then the first name and middle name. Beneath the name is the MPS venue, city/municipality name. 2. Only the official DFA passport application forms (last revised on Oct. 07, 2017), which can be downloaded from the DFA-OCA website (https://consular.dfa.gov.ph/35-downloadable-forms/56-downloadable-forms) will be used. No other forms will be accepted. Note that there are three (3) types of application forms: (1) New Passport Application Adult; (2) Renewal Passport Application Adult; and (3) New and Renewal Application Minor. 3. CamSur PESO, upon receipt of all copies of the application forms with the attached support documents, should provide applicants an indicative time (on an hourly interval) of their appointment on the date of the MPS. To avoid overcrowding at the MPS site, DFA-OCA recommends a maximum of 150 applicants per hour. It is further recommended that applicants who are senior citizens be given priority and be scheduled during the first two hours of the MPS. 4. CamSur PESO must ensure that the necessary forms and documents are complete and correct prior to submitting it to the MPS Coordinator, at least four (4) weeks before the scheduled date of the MPS, for verification and pre-processing. Expected scenario during the MPS goes this way: Step One (Registration and Screening) – Applicant will claim his/her DFA-verified application form from requesting party. Requesting party must screen if the applicant has complete and correct supporting documents; Step Two (Processing/Interview) – Applicant will present his/her DFA-verified application form and all his/her original documents to DFA personnel, for evaluation/verification; Step Three (Payment) – Applicant will pay the MPS passport fee of Php 1,200.00 to DFA cashier; Step Four (Courier) – Applicant will surrender receipt of payment to the courier and pay for courier service of Php 150.00 to the applicant’s chosen address; and Step Five (Data Capturing/Encoding) – Applicant will present his/her documents and receipt to the MPS encoders. The MPS encoders will input the applicant’s personal information and take the applicant’s photo and biometrics. The application form and documents appended to it shall be surrendered to encoders and shall form p art of the Department’s passport records. Other important reminders are: Personal appearance is required for all applicants for the capturing of photograph and biometrics. Earrings, headbands, necklaces, other forms of jewelry and contact lenses are not allowed. Applicants with medical alterations must bring a medical certificate. Smiling with visible teeth is not allowed. Applicants are required to wear decent clothing for passport photo capture. Plunging necklines, sleeveless clothing, spaghetti-strapped tops, see-through tops, sando, tube tops, halters, and the like are prohibited. The schedule of release of passports is ten (10) working days from the date of processing through the designated courier service. There shall be no early release of passports. Pending passport applications must be resolved within six (6) months. Walk-in applicants will not be accommodated during the MPS. MPS applicants with prior online appointment may not avail of MPS. For frequently asked queries like PSA or NSO birth or marriage certificates, NSO issued birth and marriage certificates are still applicable and will be honored so long as the vital entries are not blurred or unreadable. Also, birth certificates that are registered less than ten (10) years ago, applicant must submit documents that pre-date the late registration such as Voter’s Registration Record with correct information and dry seal/Form 137 with dry seal or documentary stamp. Without these, applicant must produce current valid IDs and NBI Clearance. It must also be noted that only these IDs are acceptable: Social Security System (SSS)/Government Service Insurance System (GSIS) Unified Multi-Purpose Identification (UMID) Card; Land Transportation Office (LTO) Driver’s License; Professional Regulatory Commission (PRC) ID; Overseas Workers Welfare Administration (OWWA)/Integrated Department of Labor and Employment (iDOLE) card; Commission on Elections (COMELEC) Voter’s ID or Voter’s Registration Record from COMELEC Head or Regional Office; Philippine National Police (PNP) Firearms License; Senior Citizen ID; Persons with Disabilities (PWD) ID; School ID (for students); Current Valid ePassport (for Renewal of ePassport).
